Required Medical Authorization and Forms

Medication Administration

All prescription and over-the-counter medication MUST be checked in with the School Nurse and have required forms submitted. No medication of any kind may be carried by students.

Prescription Medications:

  • Must be delivered by parent/guardian to the School Nurse in original container with current prescription label
  • Requires Medication Authorization Consent Form completed and signed by prescribing physician and legal guardian.
  • Maximum 4-week supply permitted on campus
  • First dose cannot be administered at school
  • Special procedures for inhalers and EpiPens

Non-Prescription Medications:

  • Must be delivered by parent/guardian to the School Nurse in original sealed container
  • Requires parental written consent on approved form
  • 72-hour maximum without physician’s standing order

Sports Participation Requirements

Sports Physical

Students that participate in any after-school sport (basketball, cheerleading, running/ walking, soccer, etc.) MUST submit this sports physical document completed and signed by your child’s physician. A new sports physical is required each school year.

 

Concussion for Sports Course

The State of Florida requires all school coaches and student-athletes to complete the the free NFHS Learn Concussion in Sports Course annually. Certificates of completion must be kept on file at the school.

Any child that plans to participate in any afterschool physical activities or sports at Pace Brantley Preparatory School will be required to complete the NFHS Concussion course. Children that are unable to complete the course on their own must complete the course with an adult and submit the certificate of completion before being permitted to participate in any afterschool sports activity.

Steps:

  • Go to:https://nfhslearn.com/courses/concussion-in-sports-2
  • Select “Courses” from the menu at the top of the page.
  • Select “Parent” on left side
  • Scroll down to “Concussion in Sports”
  • Select Florida in the dropdown menu and order free course
  • Create an account
  • Watch video (approximately 10 minutes) and take quiz
  • Submit your completed certificate by uploading it below.

**You are required to complete and submit the Concussion in Sports certificate each school-year.

 

Electrocardiogram Clearance

Effective July 1, 2026, all first-time high school athletes must have an Electrocardiogram (ECG) screening before participation. This applies to students with no cardiac symptoms. Students with cardiac symptoms should consult their healthcare provider. An ECG completed within two (2) years prior to July 1 of the participation year satisfies this requirement. If the ECG requires further evaluation, the student must be cleared by a licensed medical practitioner trained in the diagnosis, evaluation and management of ECGs before participating in athletic competition, practice, tryouts, or workouts.

Who Must Obtain an ECG:

  • All incoming 9th-grade student-athletes (2026-27)
  • Any student in grades 10–12 who has never participated in high school sports

Who We Play For is a non-profit organization that offers minimal cost ECG screening clinics across the country throughout the year, but frequently during the summer.
